Telehealth Appointments

We Can Continue Your PT Visits
It looks like COVID has largely become a thing of the past and we are seeing patients in the clinic.  However,  Evolve does offer telehealth physical therapy and gym appointments. Here is how it works:

• The appointments will be done via Zoom. Zoom is a free downloadable app that allows you to communicate with your physical therapists using a video format. Go here to download the app. In the upper right hand area of your screen you will see SIGN UP, ITS FREE. You will provide your email address and create a password and then you are ready to go.
• The telehealth appointments will be 30 minutes in duration for existing patients and 60 minutes for new injuries and evaluations.
• All Gym telehealth appointments will be 30 minutes.
• You will call our office at 858-457-3545 and schedule your appointments.
• Your therapist will email you a link to join your physical therapy telehealth session. You will receive this email well in advance of your appointment.
• At the time of your appointment you will click on the link and be connected directly to your physical therapy session with your therapist.
• The telehealth service option may not be covered by insurance. Therefore, Evolve has created a cash pricing package of $510 for 6 sessions. This is an investment of $85 per session. Single session pricing is $95 per session. We will collect payment prior to your first session.
• New Injury and New Evaluation sessions are 60 minutes in duration and are priced at $140-215.
• All patients (Medicare and other insurance) will be sent a waiver to sign acknowledging that you have chosen to participate in the cash-based telehealth services.
• Of course we acknowledge that telehealth doesn't allow us to use the manual techniques that we know are beneficial in progressing your care, but as soon as the dust settles from the Coronavirus we will go back to seeing you in the clinic.
• We are continuing to see physical therapy and gym patients in the clinic. We are strictly following all the suggested guidelines of hand-washing, wiping down all surfaces before, during and after visits, wiping down the entire clinic once per hour, and keeping plenty of distance between people.​
